When you get one of these sets please read line 14:
"If you have previously filled out a card like this-name of employer-place-date-number"
This is asking if the person filled out another card- not the card they are applying for with this company so this date should not be recorded. The date signed is on the left side of the application.

I have seen the "race" as "Western Samoan" and "Welsh" when it should be "White"
Take a second and review what is entered before submitting the image.
