I believe it shows the closest relationship and depending on exactly how the descent runs (for instance-one married a cousin and the other one didn't, etc.) it means that one of your 5th great grandfathers is also your 4th great uncle- and since a 4th great uncle is a closer relationship than a 5th Great grandfather is shows that relationship.
