Query about ordering Death Certificates

Hello,

I was wandering if anyone can assist with my query?

I am trying to order a copy of an ancestors death certificate on the GRO website.
I have the information for the death certificate from Ancestry.

It gives me my ancestors name, birth date and date of death registration. It also gives me the registration district, the inferred county, register number, district and subdistrict number and entry number.
When I go onto the GRO website to order a copy of certificate, one of the mandatory boxes asks me for a District name and number.
I know the district name is Wandsworth, but I am not sure what to input for the District number?
Would the district number be the 'district and subdistrict number' or the 'register number?'

Hope this makes sense?!

Also does anyone know if a female persons maiden name is always included on a death certificate?
