OA-C790 (Old Age Form) -- Help Deciphering some of the codes/abbreviations

This is for my great-grandmother - I requested a copy of her SS-5 form, but apparently this form is all they have for her.

Some of the information I can make a pretty educated guess about, I know she was in a mental institution, I am assuming the next entry means that "all known identification information is given".. what I am wondering about is the short-hand before that.

"MI Y PR ERROR UNIT PC" and also the Type of Claim "L" code..

I am guessing the TYPE OF ACTION code "RET" means "retired".

Anyone dealt with this form before or have any guesses for me?

Dear Kandy, Did you ever find answers to your questions? I have a similar form with many of the same abbreviations. My questions are:
1) What is code 181?
2) Type of action: RET
3) Type of claim: L
4) Remarks: MI U Institution
I wrote directly to the SSA but got no response.
