How can a person be listed with 2 SS numbers?

Researching a person who died 15 years ago, may be a relation. The Ohio SSDI gives one SS number, while the US SSDI gives another. Same name, birthdate, death date, death place, etc. The number in the Ohio SSDI is an Ohio number, but the US SSDI number is from a different state. Even if she lost the original card, they wouldn't give a new number for a state I doubt she ever visited. Can I get copies of the applications? Was there some fraud going on?
