Here's my attempt, based on the previous responses. Words in parentheses are guesses. (Well, a whole LOT of these are guesses! ;) --------------------------------------------------- Rheumatism Right shoulder slightly swollen, tender and limited in motion (one eighth?) No other evidence of rheumatism.
Disease of Heart Area of cardiac dullness ? ? Apex ? is found at fifth intercostal ? just within (mammary?) line. Rhythm - irregular heart (action?) feeble. A mitral systolic murmur is heard. No dilatation or hypertrophy. Considerable dyspnea. No ischemia or cyanosis.
Lungs No abnormal sounds are heard ? within lungs.
Liver Upper ? corresponds with sixth - rib, lower with tenth and does not extend below the ribs.
Kidneys Urinary analysis. Color amber, reaction acid, sp gr (specific gravity) 1.020. No albumin. No sugar. (Skin?) clear. No ? ? or ?. No (anemia?) Palms of hands somewhat calloused.
Vicious Habits No scars are ? or ?. No enlarged glands, no (substantial?) tenderness No ? (patches?) or (bony?) ?. No evidence of vicious habits. No other disability found to exist. We find that the aggregate permanent disability for earning a support by manual labor is due to rheumatism and disease of heart? not due to vicious habits and warrants a rate of $6 a month.
