Hello all,

I need some help with the writing of the attached document. I can usually decipher the writing but this one escapes me. Any help you could provide would be greatly appreciated.

I can't make out all of it, but hope this helps:

Rheumatism, disease of heart
Has done no work for ten months.
Rheumatism 1900, disease of heart 1890

Birthplace: Newtown, Conn.

Rheumatism
Right shoulder slightly smaller, tender and limited in motion one eighth. No other incidence of rheumatism.

Disease of heart

Lungs

Liver
Upper border corresponds with sixth rib, lower with tenth, and does not extend below the ribs.

Kidneys
Urinary analysis. ___ ___ reaction and st. gr. 1020 albumen. No sugar. Urine clear. No local ___ ___. No ushering(?). Palms of hands ___ white callus ___.

Vicious habits
No scars on genitals or groin. No enlarged glands. No subst___ tenderness. No mucus patches or bony nodes. No ___ of ___ ___.

No other disability found to exist.
We find that the aggregate permanent disability ___ ___ a ___ manual labor is due to rheumatism and disease of heart. Not due to vicious habits, and warrants a rate of $6 a month.

Here's my attempt, based on the previous responses. Words in parentheses are guesses. (Well, a whole LOT of these are guesses! ;)
