WHAT DOES BENEFIT PAID TO ZIPCODE 33019 MEAN??????
Is that a reference to the zipcode to which her regular retirement benefits were being mailed to??? Or is that a reference to where some sort of death or minors ss benefits were mailed??

I have contacted BOTH NY and FL for her death certificate. Nobody has her. Really bizarre

It means that's the last location the social security benefit was paid to. Perhaps she opted for direct deposit and this was the last location on file.

Consider she may have been in another state on a visit when she passed, a death cert is filed in the state where the death occurred.

Thanks for your reply. I know her to have resided in Florida for many years. NY is on record as her zip code resided. Lets say she was there visiting... would they simply record the zipcode of the hospital at which she died?

I realize it shouldnt be this difficult to find someone. She apparently has several variations of her name depending on what your reading, drivers licence, legal documents, ss, etc. But they're all the same person!! I provided NYHEALTH her ss, dob, dod and all name variations. I paid that TWICE. They dont have her!!!!!!! I also noted she has 2 variations of her dob. She was foreign born.

My next options are the following
1) pay to obtain her immigration records
2) pay to obtain her social security records
3) pay florida to search for her death certificate (even though ss says ny has it. and even though none of the online searches says she died here

????????

Assuming that no one in your family recalls the circumstances of her death, there are several other avenues you could pursue:
1) Check the cemetery where her husband was buried to see if she was buried with him. If so, they may have info about where she died.
2) Look for a probate record in Broward Co. (and possibly in Queens). Modern probate records usually indicate where the person died.
3) Look for an obituary or death notice in the Fort Lauderdale newspaper. It may indicate where she died.

I know others have said this as well, but nothing in the SSDI record indicates where the person died. The "last residence" shows the last address of record prior to death (which could be the address of someone else with power of attorney), and "last benefit" shows where the death (burial) benefit was paid. So they can suggest areas to look for a death record, but don't say anything about the death directly.

And she could have died in another state. That's why I suggest the sources above.

The SSA death index makes no reference to where a person died at all, even calling the 800 number wil not get you that information. All they list is last residence on record with them and where a last benefit was sent. Even with 1990s ATM bank accout access one could travel and stay without changing banks or bother to change addresses with SSA. If she was traveling between NY and FL then you need to check all the eastern seaboard states she would have traveled through. In fact her SSA date of death may not be her actual dod. IF she had checks coming in the mail and not electronic deposit, then if 3 months went by and the checks not cashed she would have gotten a notice to appear at an SSA office or Benifits would be stopped. The date the benefits stopped would be her "death" date in SSDI. IF she was not carrying ID or body could not be identified IF something fatal happened to her way from home you may never find a dc.
I know this because I have 2 relatives that had last addresses and last benefits addresses but both died in different counties that their SSDI information. And both are buried in cemeteries in completely different counties that SSDI and death counties. One died 3 months before his SSDI death date as the county cornerer didn't know they were on SSI as they were under the age of 60, so they didn't notified SSA and they stopped benefits after 3 months of no checks cashed and missed required appt.

WOWOWOWOW!!!

I have no reason to believe she would have ever traveled to NY in the past 25yrs before her death... except for the NY reference on her SS entry. As NYHEALTH doesnt have her... i will next check with Florida. Not only is the DOD unreliable as you say, its also possible that she died much earlier and someone committed fraud by continuing to cash her checks (in NY).

Do you agree that my next move would be FLHEALTH or do have another ideas???

Does this help? You now know where she died and can go from there? I'll see if I can find her burial place. Maybe she is buried with relatives.
