Since you know a DAR Member hopefully you have her DAR Number, you may obtain a copy of her DAR application by sending a check for $10.00 payable to Treasurer General NSDAR. You might want to also specify that you want also her supplemental papers too. If you go to http://www.dar.org and at the bottom of the page is the phone number to call. Do you know the name of her chapter? They might be able to help you with all the names of the supplements she might have sent in to NSDAR.

Mail the check along with the information to:

Registrar General NSDAR
Administration Building
1776 D Street NW
Washington, DC 20006-5303.

Requests for record copies may take six to eight weeks as they are very busy.
