The information that you see on the application form is all that was required in order to apply for a Social Security number. The only documentation required for an immigrant was to prove the applicant's identity, age, and right to work in the U.S. (e.g, a green card or work visa). The applicant didn't need to provide a "reason" for applying - only prove that they were eligible to do so.

The information you are looking for should be on your father's birth certificate. To get a copy of his long form birth certificate, you will have to apply to the Ontario Office of the Registrar General and provide a copy of his death certificate:
