For the shield that you provided as an illustration, "Gules a Fess Argent between fifteen Bezants five and four in Chief three two and one in Base." should be close enough.

I really can't make out the details on the carving of the full achievement - the fesse certainly doesn't look all that Argent!

Regarding the services of a good heraldic artist - just how much are you willing to pay? What artistry is wanted, just the shield or the full achievement - if the latter, you would be required to provide further blazon for everything besides the shield.
