When I got my book about 2-3 years ago I called and got an answering machine, left a message and 2or 3 weeks later I got a reply. It seems to be a family venture. I advise contacting all three addresses. Surely one of them is still good.
I think the cost was about $60 plus shipping but it has been worth every penny. It is hard bound and has withstood 2+ years of hard use without any wear and tear.
