Form 3903 (Rev. 9-00) has:
DECEDENT'S Name, maiden name, sex, date of death
RACE, ORIGIN OF DECEDENT, DATE OF BIRTH, AGE, COUNTY
place, residence
STATE AND COUNTY OF BIRTH, CITIZEN OF WHAT COUNTRY, MARTIAL STATUS, SPOUSE, ARMED FORCES.

PARENTS OF DECEASED, WITH MOTHERS MAIDEN NAME.
INFORMANT AND THE RELATIONSHIP OF INFORMANT TO DECEASED.
DISPOSITION OF BODY. PLACE WHERE BURIED etc
Plus all the regular info.
