If you were born in Ohio and adopted prior to Jan 1, 1964 you are entitled to your original Birth Certificate.

I have an affidavit I can email you and you can copy it and fill it out according to their instructions and mail it to the address on the bottom of the form.

Many people are able to find their birth family by getting the original birth certificate.
