The submarines were commemorated in different States, one submarine per State, except for California which had two.
All of the States in which there are memorials for various WW2 submarines, and the name of the submarine in each State are listed on this link.

He is listed on the muster rolls of the U.S.S. Swordfish and in his commemorative records as
Ray Holland 3467957 Motor Machinist's Mate Second Class, ( his final rank ), U.S. Navy
He enlisted 10th April 1940 in Little Rock Arkansas, so he was a prewar volunteer.
He joined the U.S.S.Swordfish on 10th April 1943.
There is a years difference in his official date of death in 1946 and the approximate date of the sinking of the U.S.S Swordfish in 1945, that's because when someone was listed as being missing in action, if no report was received about them after a year, they would be presumed to be deceased and their official date of death would be deemed to be one year after the last date on which they had been known to be alive.

Presumably his posthumous Purple Heart and his campaign medals were issued to your mom. ?
I believe that his name has been listed as Ray Holland on various records because that is how he was, for some reason, listed on the muster lists of the U.S.S. Swordfish, but he should have been listed by his full name.
If you want to, it's possible to have those commemorative records amended to show his full name.
It's also possible to get a copy of his service record, if you haven't already got one.
