Looks like R Reeds (or Ruds) Tel Enqy 11/11/31 – someone named R Reeds (or Ruds) made a telephone enquiry about the person/entry on 11 Nov 1931.
Soldiers who received the King’s South Africa Medal must have also received the Queen’s South Africa Medal and his record does state he received both medals.
Checking my copy of the QSA Roll from the National Archives, he’s listed with same number 5723 (you put 5623 in your message) but the surname appears to be Hefferman and the same Tel Enqy remark appears on that roll too.
I don’t know how Ancestry has the surname on QSA roll transcribed, but as you didn’t mention the QSA roll I will assume it is transcribed as I see it in the roll, in which case the remark probably relate to someone checking his details because of the different surname spelling.
Note: although there is no full list on the Leinster Regiment’s page, the general Name search at
http://angloboerwar.com/ does list both surnames (no number) as serving with the 1st Battalion LR and gives the source as the Medal Rolls.
