SS5 and Random Names

I actually have two examples of this -

My grandmother's maiden name was Veto. I had so much difficulty finding her family in census records until I searched by first name. I found them - right where they should be, with a last name of Guidone. Odd, I thought. Go back ten more years - still Guidone. Hmm...maybe Veto isn't right. I found my great-grandfather in SSDI listed as Francesco Guidone. Aha! I thought. Maybe if I get his information from SS I can see where the confusion occurs. Naturally, instead of clearing anything up, I am more confused, as the name on the application is VETO.

Second, I requested a printout and application for a cousin. The printout of information clearly lists him as Luigi Raimondi (also thus in SSDI) but his application clearly states Louis Raymond.

Where might Social Security be getting these names if not from the application, and do I have the ability to access these items?
