I'd forgotten that there were many Amish in St Mary's County (and I suspect other places), but when I was working in that area a number of years ago, there were but a few. Right now with the development of land in Southern Maryland, which has become a suburb of Washington, the only ones who are raising much tobacco are the Amish. It would be interesting to see what their medical care is in that area, and whether there are physicians there in whom they have confidence. In order to get whooping cough, the herd immunity must be pretty low.
