I apologize for being so lax this morning. I should have let you know that you may send for a copy of the application for your patriot. All you need do is send a check made payable to: Treasurer General NSDAR for $5.
Send the information on Patriot and check to:

REGISTRAR GENERAL NSDAR
Attn: Record Copy Dept.
1776 D Street NW, Washington, DC 20006-5303

It is now taking 6 to 8 weeks for a copy.
