David, have you tried clicking on "Board Information" on the right on this screen? You can then click on the link to Catholic Record Society of the Diocese of Columbus. When you reach the site you can click on the Research & Records tab and near the bottom of that page is a link that will take you to a form where you can request a lookup (Our Research Committee & Private Library).
