P.S. There is a form that you can request, from the address above, it's called an Application for
Disclosure, fill out and send in. (has to be signed and noterized). If you are the Adoptee, ask for the Adoptee Application for Disclosure, and if the Birth Parent, ask for the Birth Parent Application for Disclosure, and Birth Sibling ask for the Birth Sibling Application for Disclosure.

** When mailing off for the Application for Disclosure, be sure to include your name, and mailing address and what application you need (exp., Adoptee, Birth Parent or Birth Sibling). For anyone that went through Virginia Department of Social Services, for their adoption.
