* The witness name, at the time of the Entrustment Agreement, was (unknown) S. Kim, RN.
* The Entrustment Agreement was dated September 6, 1977.
* Age at adoption is unknown. Found out recently that Dawn was adopted out of the state of Virginia (location unknown).
* Mothers name at the time was Candice D. Harms-Lazo, Fathers name was Franklin D. Lazo.
