A person applying for a Social Security card fills out an application called SS-5, and the information asked includes birthplace and names of parents. You may or may not need this information.

To apply for the SS-5 form of Norma MAYO, born Apr. 4, 1919, go to :

then print out the letter displayed on your screen, and submit it with the fee of $27.00 to the Social Security Administration. Then wait a few months for it to arrive.
