Hi, Mackie, Affidavit what info do I need? My Birth cert. reflects, I was born in Chattanooga,Tn, showing Birth parents as Robert l Cox, & Nina L Cox, all hand written. I did not know if State locations can could bee changed I was born at local hosp, 10/01/60, no time was reflected. I was told I was premie 2lbs, 4oz and had to stay in hospital for months?. I can not find any such evidence only family repeating it, no pictures. But I know there are relatives, every where, OH, , IL,MS, TN, GA,TX Ca, AL,AK etc. I came across, an articles, that reflected an accident, seem like the name Beasley, Smith, Mather, hard to recall, anyway it noted One of the men/ someone yr of b-1902, one 1912, 0r 1917 but they had a baby Cox, in that car, 1960. It seemed strange since their was not a Cox in that car of older adults. Dont no if dob is changed, i believe my has been. So not really sure. I had in shoe box, but has disappeared. Maybe worth a try.
