I happen to own a Medicare directory published in 1973, which gives the address of South Nevada Memorial Hospital: 1800 W Charleston Blvd, Las Vegas NV 89106. It doesn’t say if “South” is the abbreviation for “Southern”.

You or your current physician could apply for your medical records. Assemble whatever information you have: date of birth, birth name if known, race, sex, time of birth, weight, name of attending physician, etc. Some of that information might appear on the “full copy” version of your amended birth certificate---the document that contains the names and ages of your adoptive parents.

Amended birth certificates sometimes contain the wrong name of attending physician---they fool you by using the name of the current head of the Dept. of Health, who happens to be a doctor.
