My mother - Marilyn Marie Dale - may have delivered a child that was given up for adoption in the summer of 1939 at the Industrial Home or other such location in Nebraska. I have also inquired at the State of Nebraska offices and was given a form to fill out and send back with proof of my mother's death. They said they would place that form in her file if there is one so any adoptee who may be connected to my mother could access that form and contact me.
