Unless there is a different registration filed, there is only one copy per person kept on file. I have found that the certificates have changed a few times - due to internal office issues. At one time, I requested a copy of a bith/death certificate through the Douglas Co health dept, and the form was on a normal sized sheet of paper. Later, went for more certificates, and they were at the time being printed on sheets 1/3 the size. Then I tried for certificates through the main health office in Lincoln, the sheets were full sized, but there were differences in the formats on what info was included. So, in your case, it is most likely a matter of where the info was filed and where it is being printed up for a copy. To ensure similar info or at least the complete info, get your copies from Lincoln. In Douglas, if they still allow it, you can view the file for info without actually getting a copy - this comes in handy for certs they can't issue yet.
