REQUESTING A MICHIGAN DEATH RECORD
The Michigan Vital Records office has records of deaths that occurred in Michigan and were
filed with the state since 1867. Some of the records were not filed with the state;
more records are missing from the pre-1916 files. Anyone is eligible to request a copy of
a Michigan death record if the application is completed and signed and the required fee paid.

Q. Where do I go to get a birth certificate or death certificate?
A. The Detroit Health Department, Vital Records Division, Herman Kiefer Health Complex, 1151 Taylor (zip 48202-1732) near the John C. Lodge expressway. The phone numbers are 313-876-4049 [recorded message] and 313-876-4133. Person must have died in Wayne County, Michigan & within the city limits of Detroit. Cost is $20.00. Call first before ordering as pricing may have changed. Hours are 8 ā 4pm Eastern time SHARP Mon - Fri. FREE parking. Cash, certified check, money order are accepted. Phone lookups will not be done. A walk-in service is available. Public browsing of the records is not permitted. Make payment to ā City of Detroit. DO NOT REQUEST A DEATH CERT. FOR A MARRIED WOMAN USIN HER MAIDEN NAME.
Personal checks or credit/debit cards are not accepted.
