Just email the Enoch Pratt Library at per@epfl.net. Give them the name and date of death along with your snailmail address. They will mail a copy to you usually within 2 weeks. You can order the first 10 for free.
Even if you only had the month and year they will still search but it will take a little longer depending on how busy they are.
