Hi,

The only Chicago Catholic church desingated as a Czech church was St. Procopius, 1641 S Allport St, Chicago, 60608, 312-226-7887. It opened in 1876, and is located at 1230 W and 1641 S.

Obviously not every Czech Chicagoan attanded that one parish but it is the only one of 495 parished to be called "Czech".
