I just bumped into your old post and I may have an answer for you:

I don't know what year the funeral was held or what Mr. Blei's ethnicity is, but there was a church very close to the adress on Avers that you gave: Epiphany 2524 S. Keeler Ave. I could imagine someone hearing Epiphany over an old telephone and putting St Tiffany (which has never existed in Chicago). Epiphany was founded in 1901 so I hope this fits the time frame.
