Social Security Form SS-5, application for SSN, has considerable genealogical information, should you want to know name of father, maiden name of mother, street address, employer, signature of Applicant. You should find Social Security Number on death certificate, making the fee for Form SS-5 less expensive. The listing did not seem to be recorded on SSDI, not unusual.
