Would anyone be able to decipher the missing information on this baptismal record?

I am trying to find the name of the chapel that GGGram Anne Morrisroe was baptized in. The family was from Tonregee (4 miles northwest of Ballaghaderreen on the N5 towards Charlestown).

I've attached a picture of the barely legible record of the front page of the book although I have the complete record for Anne's baptism.

I overlayed a translation for others in my family to be able to read but a couple things I just can't make out and don't know the area well enough.
