Fydor Dostoyevsky wrote a novel called The Idiot about a man who was epileptic, a quality that Dostoyevsky shared with this character. It appears that in the 19th century, this is what being an idiot meant and could explain the insane tag as well. I am sure epilepsy was poorly understood at the time.
