Death certificates for Leeds

1. How do I go about getting a death certificate for Abraham Rogers b. May 2, 1874, died July 11, 1961 and/or J.R. Rogers born 1872, died 1963? They share a gravestone in a Gananoque area cemetery. Of course I didn't write down which cemetery. I'm fairly certain they lived in Rockport.
2. What information is on a death certificate from Leeds County, Ontario? I got one from Simcoe County and it had the deceased's father's place of birth. Would the death certificate from Leeds have that also?
3. I'm also trying to find a death certificate for O.D. Rogers b. 1837, d. 1894 or 1896. He's buried at Ebenezer cemetery in Lansdowne. Were records kept back then?
4. Does Leeds County still exist or is it considered Grenville county?
