I show twins by putting "Twin" in the name suffex area. Then it shows up on the family page and in the name index. It also show up in reports as "John Doe, Twin," which may or may not be what you want.

If you go into the "Child View" at the bottom of the screen it has a place to put "twin/adopted/step" etc and it shows up on reports.
