How to obtain Death Certificate & Other Records

First of all, you may be able to obtain a Death Certificate from the Anaconda-Deer Lodge County Courthouse. The fee is $1.00 for all death certificates 30 years or more. Please include a stamped, self-addressed envelope. Provide the exact date of death if possible, or at least the year.

The cost of a Death Certificate from the state, however, is $10.00 and includes a 5-year search.

You can also find out more by phoning Montana State Hospital at Warm Springs:

PHONE: (406) 693-7000 and ask for Medical Records. Explain who you are looking for and ask for your grandfather's Patient I.D. number. After you obtain the I.D. number, ask to speak to the Patient Cemetery supervisor--this person can tell you if your grandfather is buried there. Once you have the Patient I.D. number, you can contact the Montana State Historical Society & Archives:

The above phone number is for the Archives Reference Services department which has in its archives all of the old Montana State Hospital patient records. This is where you need to provide the Patient I.D. Number that you got from the hospital. You must be willing to sign a confidentiality agreement, then they will send you selected records from your grandfather's records--they may even include a photograph of your grandfather. These are very reasonably priced at 15 cents a page!

Surnames: DeMangelaere, DeMangelaer
I am trying to laocate my grandfather's death certificate, I have information that he was in the state hospital when he died and is buried in Warm Springs-Anaconda-Deer Lodge Co., Mt. He died October 2 1922 His Name: Leonardus DeMangelaere, His name maybe Lenard. , I hope some one can help me.

Hi...My name is Ken DeMangelaere. I am sitting here with my father, Larry DeMangelaere. We are obviously related. I am sorry, that I dont know Leonardus DeMangelaere, but we are obviously related. Lets talk, okay?
