First of all, you may be able to obtain a Death Certificate from the Anaconda-Deer
Lodge County Courthouse. The fee is $1.00 for all death certificates 30 years or more. Please include a stamped, self-addressed envelope. Provide the exact date of death if possible, or at least the year.

The above phone number is for the Archives Reference Services department which has in its archives all of the old
Montana State Hospital patient records. This is where you need to provide the Patient I.D. Number that you got from the hospital. You must be willing to sign a confidentiality agreement, then they will send you selected records from your grandfather's records--they may even include a photograph of your grandfather. These are very reasonably priced at 15 cents a page!
