I am having trouble figuring out what this says. It's a discharge paper from 1865... civil war maybe??? Im especially having trouble reading the name and reason of discharge. Thanks to anyone who can help!!! :)

This is a discharge from the War Between The States, 1861-1865. Name appears to be Nathaniel Duke or Doke. Should be easy to find him in the Indiana military roster. He was discharged in Huntsville, Alabama, in accordance to instructions from Medical Department.

Surnames: Dokes
From his military pension files I found the following information. Nathaniel Mattox Dokes, private company F 149th Indiana Infantry. Died 27 May 1919 in Garfield, Arkansas. He filed for his invalid pension on 7 December 1875. After he died his widow applied for his pension on 28 June 1919. I hope this helps.
