Death Certificate # 6236, Webster Co., Providence, Ky. Sallie Kane Fugate, born Dec. 28, 1859, died Feb. 2, 1937 in Providence, Ky. She is listed as a widow. Husband's name Wesley Fugate. The name of cemetery is not listed on the death certificate, only information is she is buried in Providence, Ky. Hope this helps.
