I have a birth certificate for my grandfather that looks different from the birth certificate of his brother, yet they were born only two years apart in Omaha. The one I have from my grandfather was issued by the Omaha Health Department, yet the other one I have was issued by the Department of Commerce and Labor, so the obvious question is are there two of each?
The one issued by the Health Department includes only an address of birth, while the one from the Dep. of Commerce and Labor includes the residence of both the mother and the father, including the address of birth.
