Private John V. Tyack of Captain Anthony L Voughan Company C of the NYS Vol Regiment of United States Infantry was enlisted by Lieut W W Web at New York City on the seventh day of November 1861, to serve three years, he was born in England in the State of England is fourty three years of age, five feet seven inches high, fair complexion, brown eyes, grey hair, and by occupation when enlisted a carpenter. During the last two months said soldier has been unfit for duty 60 days. Private John V Tyack was perfectly healthy at the time of enlistment and has always performed his duty faithfully. His disease was contracted by exposure and heavy marches during our march through Louisiana in the month of May 1863.
Station: Donaldsonville, LA
Date: July 24th 1863 Lieut Conrad D Ludeke, Commanding Company. I certify that I have carefully examined the said Private John V Tyack of Captain Noughans Company, and find him incapable of performing the duties of a soldier because of haemorrhoids, old age and general debility.
