Sandra M. Slyk, 76, of Howland Township, Ohio entered into eternal life on Monday, February 9, 2015 at 4:42 a.m. with her loving family by her side at Covington Skilled Nursing and Rehabilitation Center in Columbiana, Ohio following a courageous battle with Lou Gherig’s disease.

Sandra was born on June 21, 1938 in Warren, Ohio, (Trumbull County) the daughter of the late Peter and Kathryn (Kostyshak) Zackeroff and was a lifelong area resident.

A 1956 graduate of Howland High School, Sandy was a homemaker and a member of SS. Peter & Paul Byzantine Catholic Church in Warren, OH.

Sandy is survived by her loving family. Her husband, Richard “Dick” Slyk; two sons Dr. Michael (Dr. Tina) Slyk of Howland and David Slyk of Pittsburgh; a brother Richard (Sandra) Zackeroff of Warren, OH; four grandchildren Hunter, Kaylee, Bailey and Hayden Slyk and many nieces and nephews.

Besides her parents, Sandy was preceded in death by a son Joseph R. Slyk.

Calling hours will take place on Wednesday, February 11, 2015 from 4:00 p.m. until 6:00 p.m. in the Sinchak & Sons Funeral Home, where a Panachida service will be recited at 6:00 p.m. Wednesday.

A Funeral Liturgy will be recited at 10:30 a.m. Thursday in SS. Peter & Paul Byzantine Catholic Church with the V. Rev. Andrew J. Deskevich officiating, preceded by closing prayers to be recited at 10:00 a.m. in the funeral home prior in leaving for the church.

Burial will follow in Pineview Memorial Park in Warren, Ohio.

As a sign of sympathy and condolence, the family suggests that memorial contributions in Mrs. Slyk’s name be directed to the ALS Association Northern Ohio Chapter, 6155 Rockside Road, Independence, Ohio 44131
