My family originates from Lutowiska UKRAINE also. I do realize that it is not in Poland.
I am Musiowsky in Canada.
however the spelling of the name is kind of loose.
It depended upon what the immigration officials "heard" when my grandparents were trying to fill documents etc.
I was told it was originally Musijowskyj in English or Musijovski ?
My Gido was Wasyl Musiowsky and my Baba is Maria Musiowsky.
They were born in Lutowiska Ukraine, which I believe is actually now run by Poland today.
they often told people they were from Lviv to make things easier. Lvivska.

Wasyl Musiowsky was one of 7 children, I only know that one brother was possibly named Metro Musiowsky

Wasyl's fathers name was KARL Musiowsky
Karls' Fathers name was Nikolai Musiowsky

My father also mentioned something about Novi Sambir area too or Novi Sambor?

I have also seen spellings like:
MUSIJEWSKA
MUSIJEWSKI
MUSIOWSKY
MUSIJOWSKYJ
MUSIJOWSKI
MUSIJOVSKI
