Yes -- go to
http://ssdi.rootsweb.com/cgi-bin/ssdi.cgi and type in her name: Wilborn, Shirley and her birth year 1931. In the results, click on "SS-5 Letter." A letter will automatically be generated for you -- either print it out or cut and paste it into a word processing program. Fill in the blanks. Write out a check for $27 and mail it to the SSA address.
Her SSN application should contain her full name, address, place of birth, parents' names, employer, etc.
I know it's pricey (and it usually takes some time to get an answer) but I think it's the best source for the info you want.
