Just a quick note...
If you're ordering a death certificate from the State of Washington, you don't need the parents' names. All the form requires is the person's name, approximate date of death, date of birth if known, county or city of death, and spouse if known.

So with the info you already know, you should be able to get a copy of the death certificate.
If you do send in the form, put Vancouver, Clark County as the place because he may be indexed in Vancouver OR in Clark County.
