With the help of a great researcher in PA, I have been able to find my great grand fathers Death Certificate and Obit. I believe the date of birth is wrong as earlier documents; Marriage Certificate, Son's birth certificate and census, give his birth date as 1891.

His mother was Aneila Belak and his father Leon Lewandowski, both of Poland and both deceased in 1959, but he had two siters and a brother alive when he passed in 1959.

I have ordered his SS application to verify his date of birth. What I am looking for now is his village of birth.
