I would also urge caution, should you use the maiden name on the death certificate. The information on that form is only as good as the knowledge of the person providing it. My paternal grandmother's maiden name was listed incorrectly. Her family didn't know her true maiden name. Luckily, I had a couple conflicting documents right off, so I didn't waste a ton of time tracking the wrong name. Marriage licenses can be helpful, if you can find them.
