Classification: Query
Surnames: Deceased Mom's Birth Certificate from NYC
You guys would have a lot more privacy if you emailed directly, off this permanent list.
Just sayin' Maybe I can help you guys some.

You can get your deceased parent's birth certificate, in person or by mail, even if it is less than 75 years. The place>
NYC Department of Health and Mental Hygiene
Office of Vital Records
125 Worth Street, CN-4, Room 133
New York, NY 10013

The most important thing is to document-up. You need: Death cert of parent, your Birth or Baptismal Cert, your photo ID, the CORRECT form completed, and a SASE, plus payment for cert AND fees. See>http://www.nyc.gov/html/doh/html/services/vr-order-other.sht...

SUGGESTIONS: Hand print form. Make sure you include all copies above. Don't forget payment.
During the summer you may have to wait longer to get the BC in the mail.

All they are trying to do is to make sure the important certs get to family.

Good luck!
