If you decide to send for the death certificate let me know. Because I live locally I drop my requests off at the dept. of health and get the certificate in the mail in about 2 weeks. Mail in requests take months. Don't know why but that's just the way it is. NYS charges $22 per certificate (highway robbery I know) so you would have to send me a check/money order for $22 (made payable to NYS Dept. of Health/Vital Records) along with the completed form and I would drop them off for you.
