In regards to the birth and death records, they should be on file at the Trenton Archives, but to order the record, you have to have an exact birth or death date....
Sometimes that requirement is self-defeating! But I know someone who spends a lot of time there, so I'll see what I can find out.
(Trenton is the State of New Jersey's capital)
