I found the following in the Maple Park Cemetery book published by the Ozarks Genealogical Society:

Block 15, Lot 8

Walter Mitchell, died 14 Jan 1895, IOOF symbol
America Mitchell, w/o Walter Mitchell, died 1 Oct 1875, age 46 yrs, Mother
Henry C. Mitchell, died 15 Feb 1878, age 33 yrs

There's a note (moved from Hazelwood 19 Sep 1878) that applied to Henry C. Mitchell, I think. It's hard to tell for sure from the way it's put in the book. Since America is there and was originally buried in Hazelwood it may be for her.
